Online (MOTOR VEHICLE ACCIDENTS ONLY):

​ Please note that it takes 7-10 business days for an accident report to be completed. If you cannot find your accident report online, please contact the Records Bureau.                                               ​

In Person:

The Records Bureau hours are Monday – Friday, 9am – 4pm (except holidays).  Please call beforehand to make sure the report is complete.

By Mail:

Send written request.  You must include the date and location of the incident as well as the name(s) of the person involved.  Include a check or money order for $5.00 payable to the “Borough of Point Pleasant Beach” with a self-addressed, stamped envelope.  NO reports will be sent via Fax.

Court Discovery:

Requests for discovery may be faxed to (732) 892-0088 or mailed to 416 New Jersey Ave., Point Pleasant Beach, NJ 08742 with attention to Kellie.

Applications for Temporary Placards:

The application for a Temporary Placard can be picked up, mailed or emailed to you or be printed by the form below under forms.  Applications must be signed by a medical doctor, podiatrist, or chiropractor and returned to the Records Bureau with a check made payable to “NJ Motor Vehicle Commission” in the amount of $4.00.  These temporary permits will be issued only when necessary and when the applicant’s physical condition is not considered permanent.  The permit expires six (6) months from issue date and may be extended an additional six (6) months.  Permanent Placards can be obtained through NJ Division of Motor Vehicle.

Note: Portions of the report may be “redacted” (blacked out) by law. The information redacted is sensitive information concerning involved parties, such as social security numbers, dates of birth, and in some cases driver’s license numbers. ALL juvenile information (including names) will be redacted from reports as well.
