FIREARMS APPLICATIONS AND PERMITS


ALL APPLICANTS:

1. Type or print information clearly on all forms.

2. All addresses must be complete with house number, street (P.O. Box), town, state, and zip code.

3. All forms must be signed.

4. Height shall be in feet and inches.

5. Indicate Race, either White, Black, Asian or American Indian.

6. Contact Firearms Processing Clerk (Valerie Blauvelt) at 732-892-0500, ext. 113, before dropping off forms.

7. Fee: $5.00 for ID card; $2.00 for each Permit. Will be collected when ID card/Permit is issued. Check or money order made payable to “Point Pleasant Beach Police Department”, or cash (exact change only).

* The falsification of information on any of the applications for firearms permits is a violation of N.J.S. 2C:39-10c and is a crime of the third degree. Any falsification may result in criminal charges against you. *

FIRST TIME APPLICANTS FOR FIREARMS PURCHASER IDENTIFICATION CARD AND/OR HANDGUN PURCHASE PERMIT:

1. Complete and sign Application for Firearms Purchaser Identification Card/ Application to Purchase a Handgun. Check the appropriate box at top of application. Complete the application in its entirety. Answer all questions fully and provide complete mailing addresses with zip codes for those individuals listed as references. Do not use a relative as a reference.

2. If you are applying for a “Permit to Purchase a Handgun”, indicate same on top of application and enter number of permits requested.

3. Complete and sign Consent for Mental Health Records Search (SP-66) .

4. Fingerprinting – Done by outside agency (IdentoGO). Form will be provided by police department with instructions on how to complete fingerprinting. (Please return copy of form to Point Pleasant Beach Police Department after being fingerprinted.)

SUBSEQUENT APPLICANTS FOR ADDITIONAL PERMITS TO PURCHASE A HANDGUN:

1. Complete and sign Application for Firearms Purchaser Identification Card/ Application to Purchase a Handgun. Check the appropriate box at top of application and enter the number of permits requested. Complete the application in its entirety. Answer all questions fully and provide complete mailing addresses with zip codes for those individuals listed as references. Do not use a relative as a reference.

2. Complete and sign Consent for Mental Health Records Search (SP-66).

3. A Criminal History Records Check must be conducted on all subsequent applications. The instructions for this online request can be located below.

APPLICANTS FOR DUPLICATE I.D. CARD (CHANGE OF ADDRESS, LOST/STOLEN, ETC.):

1. Complete and sign Application for Firearms Purchaser Identification Card/ Application to Purchase a Handgun. Check the appropriate box at top of application. Complete the application in its entirety. Answer all questions fully and provide complete mailing addresses with zip codes for those individuals listed as references. Do not use a relative as a reference.

2. Complete and sign Consent for Mental Health Records Search (SP-66).

3. If you possess your old Firearms Purchaser I.D. Card, submit the card with the application.

4. A Criminal History Records Check must be conducted on all subsequent applications. The instructions for this online request can be located below.

APPLICANT INSTRUCTIONS FOR ONLINE CRIMINAL HISTORY RECORDS CHECK (REQUIRED FOR ADDITIONAL PERMIT TO PURCHASE OR DUPLICATE I.D. CARD):

1. Log onto https://www.njportal.com/njsp/criminalrecords/ and click on the ON LINE FORM 212A, a highlighted block located on the lower left side of the page.

2. Enter this department’s ORI number “NJ0152500”. Correct ORI number is essential for processing this form.

3. Follow the prompts for demographic and payment information.

4. Upon completion, you will receive an email confirmation and receipt with confirmation number.
